The College Avenue Dental Membership Plan is an in-house discount plan and is not dental insurance.
Discounts apply only to services performed in-house at Smile Roots Dental. The plan cannot be combined with other insurance plans, including PPO, HMO, or Medi-Cal plans. Membership benefits and discounts are non-transferable and apply only to enrolled patients.

Limitations & Exclusions
- Periodontal disease management (Scaling & Root Planing D4341/D4342 and Periodontal Maintenance D4910) receives the 15% practice discount — these procedures are NOT 100% included.
- Discounts are suspended if your monthly payment is past due. All payments must be current to access discounts.
- Discounts apply exclusively to work performed in-house by the Smile Roots team. They do not extend to outside specialists (e.g., Endodontists, Oral Surgeons) or external lab fees.
- Benefits cover a single member only.
